No matter the outcome of the Affordable Care Act, the long-term picture for payers remains uncertain. Even if all the ACA provisions are enforced by the Supreme Court, there are still many issues to sort out. If some of the provisions are struck down, then other issues will be encountered. Getting The Right Care When It Matters Most Presentation

Almost all patients and families are willing to consider and talk about future medical decisions if they see how this effort will improve their own treatment. We must be able to explain the benefits of the effort of having the conversation with the patient/family. A standardized, patient-centered, staged approach, rather than legalistic, to these advance care planning conversations is crucial. An organized system of work flows, processes, and electronic medical record (EMR) is needed in all health care settings. The effort of ACP must be built into the routine of care and shown that it improves patient outcomes.

The evidence shows that families who face complex, moral/medical decisions are better prepared with effective advance care planning and deal with grief in a healthier way with fewer complications. This leads to not only positive feelings toward the health organization who provided end of life care, but also to fewer missed days at work.

In this presentation, Dr. Jeff Thompson, MD of Gundersen Lutheran Health System, discusses how advanced planning, an integrated delivery system, available health records, community collaboration and a not-for-profit mission results in better care at far less cost in the last 2 years of a seniors life.

Dr. Thompson is the CEO and chairman of the boards of Gundersen Lutheran Health System, as well as a practicing pediatric intensivist and neonatologist. He’s the founding member and past board chair of the Wisconsin Collaborative for Healthcare Quality and is chairman of the board of the La Crosse Medical Health Science Consortium. Dr. Thompson has authored a number of articles, book chapters and abstracts on many healthcare topics.
