Using tools from Lean Six Sigma to document and analyze the process, the team was charged with tracing the flow of a patient and caregiver and examining the current quality, cost, and outcome drivers of the existing care process. Lastly, the team was to identify opportunities to reduce costs and improve the overall quality of care for Alzheimer's patients and their caregivers.
